Vanderbilt Medical Record Forms (VMR Forms) is the information hub for all documents that will be entered into the electronic medical record. This encompasses both internal VUMC forms, as well as, non-VUMC external forms used by our staff and clinics.

All internal forms are required to meet the mandatory Vanderbilt form requirements or they will not be entered into the medical record.

External forms (non-VUMC forms) used by staff may include mandated regulatory documents or forms required by 3rd party entities completed by Vanderbilt providers. These forms will, also, need to be submitted for review. Appropriate approval committees will decide the best way for these documents to be accessed. Our preferred practice would be through a link provided in E-Docs.

If a form is requested to be built by our IT team, the owner of the form will be responsible for routinely tracking any updates made to that form to make sure it remains compliant.

Vanderbilt Medical Record Forms Primary focus is to:

  • Move the organization to a fully paperless environment
  • Review and analysis all document content and format to adhere to VUMC medical record standards and regulatory compliance
  • Accurate Naming Convention/Doc Type Nomenclature
  • Real-time inpatient/outpatient scanning
  • Assure medical record compliance in scanning and indexing standards
  • Policy creation to define standards for the use and functionality of the medical record
  • Deployment of new and revised policies
